Qualifications

  • Bachelor's Degree Pharm.D. or Bachelor degree in pharmacy required.
  • Infectious Diseases residency training or board certification preferred.
  • 3 years of relevant experience as a clinical pharmacist or residency/fellowship preferred.

Responsibilities

  • Provide professional infectious diseases pharmaceutical care for Corewell Health patients.

Corewell Health Farmington Hills opened on Jan. 19, 1965 as a 200-bed facility known then as Botsford General Hospital. Today, Corewell Health Farmington Hills Hospital is a 305-bed teaching facility with Level II trauma status.

Scope of Work

The Infectious Diseases (ID)/ Antimicrobial Stewardship Clinical Pharmacist Specialist is responsible for providing professional infectious diseases pharmaceutical care for Corewell Health patients. This service will focus on continuous growth, maintenance, and documentation of the Antimicrobial Stewardship Program (ASP) in the acute and ambulatory care setting. This will be accomplished through collaboration, coordination and communication with the clinical/operation pharmacy leaders, hospital administration, compliance leaders, clinical pharmacists, medical staff, infectious diseases physicians, infection preventionists, microbiologists, information technology and quality and safety leaders. The main goals of the pharmacist in this role include consolidation of the antimicrobial formulary; cost stewardship; promotion of the safety and accuracy of antimicrobial therapy of admitted patients to Corewell Health and improving quality metrics related to antimicrobial use. Pharmacists involved in direct ID patient care are responsible for monitoring select patient's medication therapy and reviewing their patient's medication profile, laboratory results, and clinical parameters applicable to ID. Pharmacists will practice based on evidence-based literature and approved health system policies and protocols. Additionally, individuals working in the role of an ID pharmacist will focus on initiatives to help improve and/or maintain microbiological effectiveness of antimicrobials, decrease the emergence of resistance, provide education to prescribers (including pharmacists, nurses and ancillary healthcare providers), decrease adverse reactions and predisposition to secondary infections and improve overall patient care.

Provides leadership for all Pharmacy clinical activities within assigned area. Embraces the goals/objectives encompassing Pharmaceutical activities. Promotes team efforts with pharmacy and non-pharmacy personnel to manage and achieve departmental/corporate objectives. Provides support for staff development so that staff meets/exceeds minimum competency standards and encourages/supports staff to continue professional development. Demonstrates good judgment and decisiveness throughout daily activities. Maintains knowledge of current pharmaceutical practice and demonstrates a personal commitment to professional growth. Serves as role model of clinical excellence to staff pharmacists. Supports management by providing oversight, guidance and support for all assigned areas. Participates in the planning, organization, implementation and follow up of new programs and activities. A key role of the Clinical Pharmacy Specialist is to promote safe, appropriate, and cost effective medication therapy.

Qualifications
  • Required Bachelor's Degree Pharm.D. or Bachelor degree in pharmacy.
  • Preferred residency training.
  • 3 years of relevant experience 3 years of experience as a clinical pharmacist or pharmacy residency/fellowship preferred. In absence of residency training, candidates with demonstrated superior knowledge (board certification, considerable practical clinical experience, etc.) in area of focus may be considered. Preferred
  • Infectious Diseases residency training, equivalent years of experience, or board certification preferred.
