Qualifications

• Completion of an accredited PGY‑1 Pharmacy Residency or fellowship required.
• Completion of an accredited PGY‑2 Specialty Pharmacy Residency preferred.
• 5+ years clinical pharmacy experience with no residency is acceptable.

Responsibilities

• Provide patient-centered care and integrated pharmacy services.
• Participate in all aspects of the medication-use system.
• Support centralized and decentralized medication-use systems.
