Job summary

Hillcrest Medical Center in Tulsa, OK is seeking a Day shift, full-time Float Clinical Pharmacist Specialist to join our clinical team. The role emphasizes patient-centered care, collaboration with physicians and nurses, and participation in medication-use systems across decentralized settings.

Required qualifications include a Pharm D or B.S. with ACPE accreditation, hospital clinical pharmacy experience, and eligibility for BPS certification; licensure in good standing plus BLS/ALS-ACLS.

Qualifications

• B.S. or Pharm D. from ACPE-accredited School of Pharmacy.
• Clinical pharmacy experience in a hospital setting.
• Completion of PGY-1 Pharmacy Residency or fellowship; PGY-2 in an appropriate specialty, or 3+ years of clinical experience with no PGY-2.
• Board of Pharmacy Specialties certification (or obtained within 1 year).
• Active Pharmacist license in good standing with the State Board of Pharmacy.
• BLS and ALS/ACLS certifications; other state requirements.

Responsibilities

• Provide safe, effective, and prompt medication therapy and patient-centered care.
• Deliver integrated pharmacy services in a decentralized setting with physicians, nurses, and other staff.
• Support centralized and decentralized medication-use systems within the hospital.

Responsibilities

The clinical pharmacist specialist isresponsible for the provision of safe, effective, and prompt medication therapy.Provide patient-centered care and integrated pharmacy services in a decentralized practice setting with physicians, nurses, and other hospital personnel. Inthis clinical role,the specialistparticipatesin all necessary aspects of the medication-use system while providing comprehensive pharmaceutical care to the patients intheassigned areas.As well as provide support of centralized and decentralized medication-use systems.

Qualifications

Job Requirements:

• B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.
• Clinical pharmacy experience in a hospital setting
  • Completion of an accredited PGY-1 Pharmacy Residency or fellowshipANDCompletion of an accredited PGY-2 Specialty Pharmacy Residency or fellowship (in an appropriate specialty field).
  • ORCompletion of an accredited PGY-1 Pharmacy residency or fellowshipAND3+year's clinical pharmacy experience (if no PGY-2 is completed).
  • OR 5+year's clinical pharmacy experience with no history of residency or fellowship.
• Board of Pharmacy Specialties (BPS) certification (or attained within 1 year).
• Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.
• Other applicable state requirements - i.e. sterile compounding, etc.
• BLS certification.
• ALS/ACLS certification.
• Board Certification.

Preferred Job Requirements:

• Doctor of Pharmacy from an ACPE-accredited college of pharmacy.
• Experience working in Microsoft Office applications, Electronic Medical Record Systems
