Hillcrest Healthcare System in Tulsa, OK is seeking a day shift, full-time Float Clinical Pharmacist Specialist to join our hospital team. This role emphasizes safe, effective medication therapy and patient-centered care within a decentralized setting with physicians, nurses, and other staff. Qualified applicants hold a B.S.

or Pharm D. from an ACPE-accredited school and have hospital clinical pharmacy experience; PGY-1/PGY-2 residencies or 5+ years of practice with no residency are considered.

Qualifications

• Bachelor of Science or Pharm D. from ACPE-accredited School of Pharmacy.
• Hospital-based clinical pharmacy experience required or preferred.
• Residency (PGY-1/PGY-2) or 3+ years clinical experience; 5+ years if no residency.

Responsibilities

• Provide patient-centered care and integrated pharmacy services in a decentralized practice setting with physicians, nurses, and hospital staff.
• Participate in all aspects of the medication-use system and deliver comprehensive pharmaceutical care.
• Support centralized and decentralized medication-use systems.

Responsibilities

The clinical pharmacist specialist isresponsible for the provision of safe, effective, and prompt medication therapy.Provide patient-centered care and integrated pharmacy services in a decentralized practice setting with physicians, nurses, and other hospital personnel. Inthis clinical role,the specialistparticipatesin all necessary aspects of the medication-use system while providing comprehensive pharmaceutical care to the patients intheassigned areas.As well as provide support of centralized and decentralized medication-use systems.

Qualifications

Job Requirements:

• B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.
• Clinical pharmacy experience in a hospital setting
  • Completion of an accredited PGY-1 Pharmacy Residency or fellowshipANDCompletion of an accredited PGY-2 Specialty Pharmacy Residency or fellowship (in an appropriate specialty field).
  • ORCompletion of an accredited PGY-1 Pharmacy residency or fellowshipAND3+year's clinical pharmacy experience (if no PGY-2 is completed).
  • OR 5+year's clinical pharmacy experience with no history of residency or fellowship.
• Board of Pharmacy Specialties (BPS) certification (or attained within 1 year).
• Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.
• Other applicable state requirements - i.e. sterile compounding, etc.
• BLS certification.
• ALS/ACLS certification.
• Board Certification.

Preferred Job Requirements:

• Doctor of Pharmacy from an ACPE-accredited college of pharmacy.
• Experience working in Microsoft Office applications, Electronic Medical Record Systems
