Primary Function
The Clinical Pharmacy Specialist, Geriatrics, collaborates with the geriatrics consult service, including geriatricians, hospitalists, nurse practitioners, medical residents, and nursing staff, to enhance pharmacotherapy and outcomes for elderly patients. The specialist is involved in geriatric-focused institutional initiatives through committee participation and partnerships with the quality department. Additionally, the role includes providing pharmacy oversight in the hospital-at-home program, offering pharmacotherapy recommendations, assisting with medication processing and workflow, and developing or maintaining standard operating procedures and pharmacy policies.
