Position Overview

The Clinical Pharmacy Specialist, Geriatrics, collaborates with the geriatrics consult service, including geriatricians, hospitalists, nurse practitioners, medical residents, and nursing staff, to improve pharmacotherapy and outcomes for elderly patients. The specialist also participates in geriatric-friendly initiatives through committees and partnerships with the quality department. Additionally, the role involves providing pharmacy oversight in the hospital-at-home program, offering pharmacotherapy recommendations, assisting with medication workflows, and developing standard operating procedures and policies for the pharmacy department.

Job Qualifications and Specifications

• Doctor of Pharmacy (PharmD) degree with at least 5 years of clinical pharmacy practice or post-graduate residency training.
• Current Rhode Island Pharmacist license.
• Board Certification in Geriatrics Pharmacotherapy (or Board-eligible) is preferred.
