Job summary

Centerpoint Medical Center in Independence, MO is seeking a Clinical Pharmacist PRN to join our hospital pharmacy team. You will interpret orders, compound and dispense medications, monitor therapy for interactions and dosing, and provide leadership in the medication-use system.

The role emphasizes patient safety and cost-effective therapy, collaboration with medical staff, oversight of scheduling, participation in clinical services, and quality reporting. A PharmD or B.S.

Qualifications

• Active pharmacist license in good standing with the State Board of Pharmacy.
• Minimum 1 year of hospital experience preferred.
• Completion of hospital PGY1 residency or fellowship preferred.
• At least one of: 1 year hospital clinical pharmacy experience or 2 years general hospital pharmacy experience.
• Specialty BCPS certification preferred.
• State Medications Therapy Management Certification

Responsibilities

• Interpret orders and transcribe into the pharmacy computer system.
• Accurately compound and dispense pharmaceuticals.
• Monitor drug therapy regimens for contraindications and interactions.
• Suggest cost-effective therapeutic medication alternatives to staff.
• Support formulary stewardship and rational drug therapy selection.
• Provide clinical services such as drug consults, nutrition support, anticoagulant monitoring, and pain management.
• Complete notification reports regarding medication errors and adverse drug reactions.
• Provide oversight of staff schedules.

Job Summary And Qualifications

Our Pharmacists provide comprehensive pharmaceutical services for hospital inpatients. The role of the Pharmacy is to promote safe and effective medication therapy for all patients and to provide oversight and leadership of the medication use system. What you will do in this role:

• You will interpret orders and transcribe into the pharmacy computer system.
• You will accurately compound and dispense pharmaceuticals.
• You will monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
• You will suggest appropriate, cost-effective therapeutic medication alternatives to medical staff.
• You will sustain the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and promote rational drug therapy selection.
• You will assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.
• You will complete notification reports regarding medication errors and adverse drug reactions.
• You will provide oversight of staff schedules.

What Qualifications You Will Need:

• B.S. or PharmD from an ACPE-accredited School of Pharmacy, a Doctor of Pharmacy from an ACPE-accredited college of pharmacy is preferred
• Active pharmacist license in good standing with the appropriate State Board of Pharmacy
• State Specific Medications Therapy Management Certification
• Minimum 1 year of Hospital experience highly desirable and preferred.
• Completion of hospital PGY1/ pharmacy general practice residency or fellowship highly preferred
• At least one of the following: Minimum of 1 year of hospital clinical pharmacy experience or at least 2 years of general hospital pharmacy experience preferred; within area of focus.
• Specialty BCPS certification preferred
