Job summary

UNC Health is seeking a Clinical Pharmacist Practitioner (CPP) to join the Advanced Clinical Therapeutics Program at UNC Hospitals. This role provides clinical support for patients on high-cost, complex biologics and specialty therapies with medication management and care coordination across ambulatory settings.

The CPP will work with a multidisciplinary team, supervise transitions of care, and offer education to students and residents, with an academic appointment possible based on experience.

Qualifications

• Bachelor's degree in Pharmacy is required.
• Licensed Pharmacist in NC; CPP credentialing/privileges completed before start date.

Responsibilities

• Builds a clinical practice coordinating with physicians to care for clinic patients.
• Identifies eligible patients for specialty pharmacy and supports adherence programs.
• Precepts/teaches UNC Eshelman School of Pharmacy students and residents on experiential rotations.
• Leads quality improvement initiatives and develops clinical programs within the clinic/department.
• Coordinate medication access and transitions of care for specialty therapies.
