Job summary

Qualifications

Required Doctor of Pharmacy (Pharm.D.) or Bachelor’s Degree in Pharmacy.
Licensed Pharmacist in Texas or eligible for licensure.
PGY1 ASHP residency with optional PGY2 in Critical Care/Pediatrics preferred.
Inpatient hospital pharmacy clinical experience preferred.

Responsibilities

Actively participates in multi-disciplinary rounds with medical specialty services.
Identifies patients with special medication needs and oversees coordination of drug therapy delivery.
Serves on patient care committees and implements changes to improve patient care.
Performs pharmacokinetics and therapeutic drug monitoring per guidelines.
Promotes formulary use; identifies non-formulary medications as needed.
Educates pharmacists, residents, and students; develops educational programs.
Coordinates quality improvement activities to enhance patient outcomes.
Acts as liaison between pharmacy, nurses, and physicians.
