Qualifications

• Bachelor's degree in Pharmacy and valid Pharmacist License.
• At least 1 year of related experience.
• Strong knowledge of pharmacology and drug therapy.

Responsibilities

• Directs and coordinates the clinical activities of the assigned pharmacy service area.
• Reviews physician orders for appropriateness of indication, dose, allergies, interactions and duplication.
• Contacts physicians when questions arise regarding medication orders and documents interventions.
• Ensures medication is prepared, labeled and compliant with state and federal regulations.
• Provides drug information to patients and health professionals utilizing current literature.
• May conduct medication history interviews and reconcile therapies with patient education.
• Participates in Pharmacy Quality Programs and collaborates with other pharmacists.
