Qualifications

• Bachelor’s degree in Pharmacy and Pharmacist License required.
• One year of related experience; PharmD and Board Certification preferred.
• Four years of related clinical pharmacy experience preferred, with direct patient care.

Responsibilities

• Directs and coordinates the clinical activities of the assigned pharmacy service area.
• Reviews physician orders for appropriateness of indication, dose, allergies and interactions.
• Intervenes with outcomes via Meditech Clinical Intervention routine and communicates with physicians.
• Ensures medication is prepared and labeled with required information; supervises staff and complies with regulations.
• Provides accurate drug information and educates patients and families; participates in medication education and review.
