Job summary

Jefferson Health seeks a Clinical Pharmacist to oversee the medication distribution system and verify that each medication is indicated, effective, safe, and convenient for patients. You will help prevent and resolve drug therapy problems across the department in a high-quality hospital setting.

The role includes ensuring formulary compliance, supervising staff, training residents and interns, and precepting students, with attention to diverse patient needs and safety standards in Philadelphia.

Qualifications

• Pharmacy Doctorate (PharmD) or RPH required.
• Knowledge of pharmacokinetic principles for medication dosing.
• Monitor patients for response to medications.
• Identify and document sentinel events, near misses, medication errors and adverse drug reactions in the electronic reporting system.
• Promote and assure compliance with the hospital formulary and policies.
• Provide care appropriate to the patient’s age and cultural background.
• Registered Pharmacist (PA) required.

Responsibilities

• Interacts with co-workers, visitors, and other staff in line with hospital values.
• Assures appropriateness of drug therapy and compliance with hospital policies.
• Oversees distribution of medications.
• Supports implementation of processes to assure quality of Pharmacy services.
• Trains staff (residents, interns, technicians, new pharmacists).
• Precepts student pharmacists as assigned.
