Job summary

Jefferson Health in Philadelphia seeks a Clinical Pharmacist to oversee medication distribution and ensure drug therapies are indicated, effective, safe, and convenient for patients. You will monitor responses, document events, and promote formulary adherence.

Qualifications include a Doctorate of Pharmacy or PA licensure, knowledge of pharmacokinetics, and experience with patient care in a hospital setting. Preference for residency training and familiarity with Word/Outlook.

Qualifications

• Doctorate of Pharmacy or PA license required.
• Knowledge of pharmacokinetic principles required for medication dosing.
• Monitor patients for response to medications.
• Identify and document sentinel/serious events, near misses, medication errors and adverse drug reactions in the electronic reporting system.
• Promote and assure compliance with the hospital formulary and policies and procedures defined by the P & T Committee.
• Provide care appropriate to the patient’s age and background.

Responsibilities

• Interacts with co-workers, visitors, and other staff in alignment with hospital values.
• Oversees distribution of medications throughout the hospital.
• Trains staff (residents, interns, technicians, new pharmacists).
• Precepts student pharmacists as assigned (Preceptor Development & Evaluation Program).
