Qualifikationen

• 3 years of progressively higher level graduate education leading to a Ph.D. degree, Pharm.D. or equivalent.
• Specialized experience equivalent to GS-11 in medication therapy management and direct patient care.
• Meets education requirements within 30 days of closing date.

Aufgaben

• Monitor, modify, and evaluate drug therapy to optimize outcomes; interact with patients for drug history and education.
• Perfom pharmaceutical compounding for sterile and non-sterile preparations, including extemporaneous formulations.
• Evaluate medication orders for appropriateness based on guidelines, patient factors, interactions, allergies, and formulary.
• Dispense medications with attention to accuracy, labeling, safety, and regulatory compliance.
