Responsibilities

• Accurately dispenses medications per orders and protocols.
• Reviews orders and optimizes therapy with clinical input.
• Counsels patients on use, effects and adherence.
• Collaborates with physicians, nurses and staff to resolve issues.
• Participates in medication safety initiatives and audits.
• Develops and implements clinical protocols to standardize medication use.
• Monitors outcomes and adjusts therapy to improve safety and efficacy.
• Maintains documentation in the electronic health record.
• Engages in ongoing education and quality improvement.
• Manages pharmacy inventory and cost-saving opportunities.
