UMass Memorial Health seeks a Clinical Pharmacist to provide comprehensive medication management, education, and collaborative care within our hospital. You will work with clinicians to optimize therapy, monitor outcomes, and ensure patient safety.

Includes supervision of technicians and involvement in discharge planning, education, and research activities. Massachusetts license and Pharm.D or BSPharm with hospital experience required.

Qualifications

• Pharm.D degree or BSPharm with 3 years hospital experience.
• Licensure with the Massachusetts Board of Registration in Pharmacy.
• ASHP residency (PGY1) recommended.

Responsabilités

• Provides comprehensive medication management and collaborates with care teams.
• Review patient med history, educate on therapy and monitor outcomes.
• Supervise pharmacy technicians and ensure proper dispensing.
• Support discharge planning and transitions to ambulatory care.
