Job summary

UK King's Daughters is seeking a pharmacist to provide comprehensive pharmaceutical care by reviewing and optimizing medication orders, monitoring therapy, and participating in patient care rounds. Qualifications

• Licensed pharmacist in applicable states with active credentials or eligibility.
• ACPE-accredited degree in Pharmacy (PharmD or Bachelors).
• Experience with regulatory standards and hospital pharmacy operations is preferred.

Responsibilities

• Reviews and verifies medication orders for safety and accuracy.
• Participates in medication reconciliation and clinical interventions.
• Collaborates with physicians, nurses and inter-disciplinary teams for therapy management.
• Educates patients and staff on medication therapy and safety.
• Precepts learners and supports clinical research and quality improvement initiatives.
• Maintains adherence to regulatory standards and hospital policies.
