Job summary

UK King\u2019s Daughters in Ashland, KY is seeking a Pharmacist to provide comprehensive pharmaceutical care within the Ambulatory Care Pharmacy. You will review medication orders, coordinate with physicians and nurses, and participate in patient care rounds to optimize therapy and safety.

The role includes medication reconciliation, education for patients and caregivers, supervision of technicians, and involvement in quality improvement initiatives. Licensure and ACPE-accredited degree required.

Qualifications

• Bachelor's Degree or PharmD from ACPE-accredited school.
• 0–2 years of experience in pharmacy operations with knowledge of TJC, USP 797, 795 and 800, and federal/state regulations.
• An equivalent combination of education and experience may be considered.
• All experience must be paid and in the same related field.
• Volunteer work and internships for academic credit are not counted.

Responsibilities

• Participates in medication reconciliation to ensure accurate home medication lists.
• Reviews and verifies medication orders for safety and accuracy.
• Collaborates with providers and multidisciplinary teams for comprehensive medication management and access.
• Educates patients, caregivers, and providers on medication therapy.
• Compounds, dispenses, and supervises the preparation of medications, ensuring quality and compliance.
• Maintains inventory and records for controlled substances and medications.
• Participates in quality improvement, patient safety, and cost management initiatives.
• Provides staff development, preceptorship, and professional enrichment for pharmacy personnel.
• Performs other duties as assigned.
