Qualifications

• Licensed pharmacist in TN or eligible for licensure.
• One-year hospital experience or equivalent.
• Hospital pharmacy residency preferred.
• Excellent communication and computer skills.
• ACLS must be obtained within 60 months of start date.
• BLS must be obtained within 60 months of start date.
• Doctorate degree, or Bachelor’s degree in Pharmacy.

Responsabilités

• Provide pharmaceutical care to ensure safe, effective drug therapy.
• Consult with physicians and nursing staff on patient reactions and drug interactions.
• Monitor and intervene in therapeutics to improve outcomes.
• Ensure compliance with hospital policies and government regulations.
• Assist with quality improvement initiatives, inventory and ordering of pharmaceuticals.
• Document activities using computerized documentation systems.
• Apply age-specific considerations in medication management.
