TriStar Centennial Medical Center is seeking a Clinical Pharmacist to provide therapeutic pharmaceutical care, monitor therapy, and collaborate with physicians and nursing staff to optimize patient outcomes.

The role requires hospital experience or residency, strong communication, and proficiency with pharmacy software; ACLS/BCLS within required timelines. This position supports a wide range of clinical services in a major hospital setting.

Formación

• Current TN Pharmacist licensure or eligible for licensure.
• PGY-1 pharmacy residency, PGY-2 residency in infectious diseases, and/or hospital experience with an infectious diseases background.
• One-year hospital experience or equivalent.
• Hospital pharmacy residency preferred.
• Excellent communication and computer skills.
• Advanced Cardiac Life Support must be obtained within 60 months of employment start date.
• Basic Cardiac Life Support must be obtained within 60 months of employment start date Doctorate Degree, or Bachelors Degree.

Responsabilidades

• You will utilize good problem-solving abilities, making sure that problems are resolved, calling on other knowledgeable parties as necessary.
• You will maintain strong knowledge of hospital policies and government regulations and dispense accordingly.
• You will consult with physicians and nursing staff regarding patient reactions, errors and complaints.
• You will determine and consult on possible drug interactions.
• You will assist with quality improvement initiatives, inventory and ordering of pharmaceuticals.
• You will monitor, record and communicate as appropriate utilizing computerized documentation systems.
• You will utilize knowledge of age specific needs of patient in performance of duties and responsibilities.

Job Summary And Qualifications

The Clinical Pharmacist is responsible for providing pharmaceutical care activities to ensure safe and effective drug therapy through the provision of clinical pharmacy services. The Clinical Pharmacist delivers pharmaceutical care services by monitoring and intervening in therapeutics to provide optimum outcomes in a cost-effective manner. The Clinical Pharmacist fulfills departmental responsibilities within the long-range plan, mission, vision and values of the hospital.

What qualifications you will need:
