Job summary

UW Health is seeking a Clinical Pharmacist - Overnight to join the inpatient team. The role focuses on medication therapy management within an interdisciplinary care setting, including bedside emergencies, medication reconciliation, and dosing.

Responsibilities

• Deliver comprehensive medication therapy management as part of an interdisciplinary inpatient care team.

• Engage in interdisciplinary care including medication reconciliation, therapy monitoring, and dose optimization.
• Contribute to development and implementation of clinical practice guidelines and standardized protocols.

Work Schedule

100% FTE; Night Shift. 9:00 PM - 7:00 AM. Seven days on, seven days off rotation. Hours may vary based on the operational need of the department.

We are seeking a Clinical Pharmacist - Overnights to:

• Deliver comprehensive medication therapy management as part of an interdisciplinary inpatient care team to ensure drug therapy is safe, effective, efficient, and cost effective.


• Actively participate in bedside medical emergencies, providing expertise in medication selection, preparation, procurement, documentation, and medication history review.


• Engage in interdisciplinary medical care, including medication reconciliation, therapy monitoring, order verification, patient and caregiver education, drug information support, and individualized drug dosing.


• Contribute to the advancement of pharmacy practice through the development and implementation of clinical practice guidelines, order sets, and standardized practice protocols, as well as participation in scholarly and quality improvement activities.

The ideal candidate will have inpatient pediatric experience and board certification or pediatric residency training.

Qualifications

• Bachelor's Degree in Pharmacy or a PharmD degree Required


• Completion of an ASHP Accredited Residency training program Preferred

Work Experience

• 1 year of clinical patient care and orders management experience Required


• 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred

Licenses & Certifications

• Licensed to practice pharmacy in the State of Wisconsin within 90 Days Required


• Basic Life Support (BLS) within 90 Days Required


• Pediatric Advanced Life Support (PALS) within 180 Days Required


• Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) Preferred
