Job summary

UW Health in Madison, WI, invites a Clinical Pharmacist – Pediatrics to join an interdisciplinary inpatient team. You will lead pediatric medication therapy management, participate in bedside emergencies, and educate PharmD students and residents.

The role emphasizes rounds, drug information support, individualized dosing, and guideline development to advance pediatric pharmacy practice. A pediatric residency or board certification is preferred.

Qualifications

• 1 year of clinical patient care and orders management experience required.
• 3 years of institutional hospital experience or ASHP residency training preferred.
• Inpatient pediatric experience and board certification or pediatric residency training.
• WI pharmacy licensure within 90 days; BLS within 90 days; PALS within 180 days; relevant certifications preferred.

Responsibilities

• Deliver pediatric medication therapy management as part of an interdisciplinary inpatient team.
• Participate in bedside emergencies with medication expertise.
• Teach and precept Pharmacy Residents and PharmD students.
• Engage in rounds, medication reconciliation, and dosing discussions.
• Contribute to practice guideline development, order sets, and quality improvement.

Work Schedule

100% FTE; Day/Evening Shifts. Mix of AM and PM schedules, ranging 6:30 AM – 10:00 PM. Weekend and holiday rotation required. Occasional overnight coverage as needed. Hours may vary based on the operational need of the department.

Clinical Pharmacist – Pediatrics

• Deliver comprehensive pediatric medication therapy management as part of an interdisciplinary inpatient care team to ensure drug therapy is safe, effective, efficient, and cost effective.
• Actively participate in bedside medical emergencies, providing expertise in medication selection, preparation, procurement, documentation, and medication history review.
• Teach and precept Pharmacy Residents and PharmD students through structured practical experiences and formal didactic education.
• Engage in interdisciplinary patient care rounds, including medication reconciliation, therapy monitoring, order verification, patient and caregiver education, drug information support, and individualized drug dosing.
• Contribute to the advancement of pharmacy practice through the development and implementation of clinical practice guidelines, order sets, and standardized practice protocols, as well as participation in scholarly and quality improvement activities.

The ideal candidate will have inpatient pediatric experience and board certification or pediatric residency training.

Qualifications

• Bachelor's Degree in Pharmacy or a PharmD degree Required
• Completion of an ASHP Accredited Residency training program Preferred

Work Experience

• 1 year of clinical patient care and orders management experience Required
• 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred

Licenses & Certifications

• Licensed to practice pharmacy in the State of Wisconsin within 90 Days Required
• Basic Life Support (BLS) within 90 Days Required
• Pediatric Advanced Life Support (PALS) within 180 Days Required
• Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) Preferred
