Qualifications

• Graduate of an accredited School of Pharmacy with a PharmD or BS Pharmacy degree.
• Active, unrestricted Pharmacist license in Minnesota
• Strong communication skills with the ability to provide clear, timely, and professional recommendations to physicians and nursing staff.

Responsibilities

• Review physician orders daily and verify accuracy, monitor renal/hepatic function, interactions, and dosing.
• Calculate and monitor dosing using pharmacokinetic principles to ensure safety and efficacy.
• Intervene in real-time to address medication discrepancies with evidence-based recommendations.
• Maintain thorough pharmacy records and comply with state and federal regulations for order processing.
• Serve as a primary contact for hospital liaisons and internal teams, communicating clearly and urgently.
• Pursue ongoing professional development to stay aligned with regulatory standards and clinical goals.
