Qualifications

• Pharm.D. or equivalent required.
• 3 years of graduate education toward a Ph.D., Pharm.D., or equivalent.
• Experience in comprehensive medication therapy management and direct patient assessment.
• Ability to meet qualification requirements within 30 days of opening.

Responsibilities

• Monitor, modify, and evaluate drug therapy for optimal outcomes; interact with patients to obtain drug history and educate.
• Conduct sterile and non-sterile compounding, including extemporaneous formulations.
• Evaluate medication orders for appropriateness based on guidelines, patient factors, and formulary options.
• Dispense medications with attention to safety, accuracy, labeling, and regulatory compliance.
