Indian Health Service is seeking a clinical pharmacist to provide direct pharmaceutical care to patients, manage selected chronic conditions, and support acute care needs. The role emphasizes comprehensive medication therapy management and close interaction with patients to optimize outcomes.

The position requires licensure and the ability to obtain clinical privileges. On-call duties and federal employment requirements apply.

Formación

• Three years of progressively higher level graduate education leading to a Ph.D., Pharm.D. or equivalent doctoral degree.
• Pharmacist must be able to perform comprehensive medication therapy management and direct patient assessment.
• Experience includes direct patient care, medication reconciliation, and clinical pharmacotherapy recommendations.

Responsabilidades

• Monitor, modify, and evaluate drug therapy for optimal outcomes; educate and counsel patients.
• Perform sterile and non-sterile compounding and extemporaneous formulations.
• Evaluate orders for appropriateness based on guidelines, patient factors, interactions, and formulary options.
• Dispense medications with attention to labeling, safety, and regulatory compliance.
