Job summary

UMC Health System in Texas is seeking a Staff Pharmacist to optimize patient outcomes by reviewing medications, ensuring safe dosing, and collaborating with nurses and physicians in a multidisciplinary hospital setting.

You will evaluate therapy, monitor for ADE/ADR, and participate in patient care committees while ensuring compliance with USP guidelines. A BS Pharm or PharmD and Texas licensure within 90 days are required.

Qualifications

  • Licensure to practice as a pharmacist in Texas within 90 days of employment.
  • Hospital/clinical pharmacy experience preferred.
  • Strong communication and teamwork across multidisciplinary teams.

Responsibilities

  • Promote and support the culture and values of UMC.
  • Review physician orders and patient profiles for standard, appropriate therapy and dosing.
  • Identify and document potential ADEs and drug interactions; intervene as needed.
  • Ensure medication orders are processed in a timely manner and check for accuracy.
  • Monitor patient medication profiles for allergies, contraindications, interactions, and duplications.
  • Assist in training pharmacy technicians and ensure compliance with departmental policies and USP guidelines (795/797/800).
  • Maintain proper labeling, documentation, and records with pharmacist initials.
  • Provide medication information to healthcare professionals and educate on therapy options.
