Job summary

King's Daughters in Ashland, KY seeks a pharmacist to provide comprehensive pharmaceutical care, review medication orders, monitor regimens, and participate in patient care rounds. Education requirements include a PharmD or ACPE-accredited degree, with 0-2 years of related experience and knowledge of regulatory standards (TJC, USP 797/795/800).

Qualifications

• Bachelor's Degree or PharmD Degree from an ACPE accredited school of Pharmacy
• 0-2 years of experience in pharmacy operations or related area with knowledge of TJC, USP 797/795/800 and federal and state regulations
• Equivalent combination of education and experience may be considered
• All experience must be paid and in the same related field
• Part-time and PRN experience will be prorated based on hours worked per week
• Volunteer work and internships for academic credit are not counted

Responsibilities

• Participates in medication reconciliation to ensure accurate home medication lists
• Reviews and verifies medication orders for appropriateness, safety and accuracy
• Collaborates with providers and multidisciplinary teams for comprehensive medication management and access
• Educates patients, caregivers, and providers on medication therapy
• Compounds, dispenses, and supervises preparation of medications with quality and compliance
• Maintains inventory and records for controlled substances and medications
• Participates in quality improvement, patient safety, and cost management initiatives
• Provides staff development, preceptorship, and professional enrichment for pharmacy personnel
• Performs other duties as assigned
