Job summary

The pharmacist will counsel patients and caregivers, design and monitor pharmacotherapeutic regimens, review orders, and participate in emergency management and safety initiatives.

Qualifications

• Bachelor’s or Doctorate in pharmacy from ACPE-accredited school or FPGEC certificate required.
• Immunization and non-vaccine injectable certification may be required depending on practice area.
• Credentialing and privileging required based on area of practice.
• No experience required; prior clinical pharmacist experience preferred.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens.
• Provides patient and caregiver counseling on medication therapy, adverse effects, compliance and administration.
• Reviews and interprets medication orders and prescriptions.
• Participates in medical emergencies and supports medication safety and quality improvement.
• Collaborates to manage formulary adherence and medication-use processes.
