PharMerica in Medina, OH is seeking a full-time Pharmacist for its long-term care focused, non-retail pharmacy team. The role emphasizes clinical decision-making, collaboration with physicians and nurses, and rigorous documentation to ensure safe and effective medication management.

You will evaluate orders, provide therapy recommendations, maintain resident profiles, and ensure regulatory compliance while supporting ongoing training and advancement opportunities within a growing company.

Qualifications

• Pharmacist license in the state of practice.
• Pharmacy degree required; PharmD preferred.
• Experience in long-term care desired (1 year).
• Knowledge of drug therapy guidelines and regulatory standards (CMS, DEA, FDA).
• Proficiency with electronic medical records and pharmacy information systems.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and healthcare staff.
• Maintain up-to-date resident profiles with medical history, labs, and medications.
• Ensure monitoring tests for high-risk meds are ordered and communicated.
• Collaborate with physicians, nurses, and staff for clinical support.
• Document communications and interventions in the patient record.
• Enforce policies for safe handling, dispensing, and medication management.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication-related errors to Pharmacy Manager.
• Educate clinical team on safe medication handling procedures.
