PharMerica in Medina, OH is hiring a full-time Pharmacist to join our closed-door, non-retail pharmacy serving long-term care and senior living clients.

You will evaluate orders, provide clinical recommendations, maintain patient profiles, and collaborate with physicians and nurses to optimize therapy. This role offers professional growth in a growing national network.

Qualifications

• Must hold a current and valid pharmacist license in the state of practice.
• Experience in long-term care settings preferred.
• Knowledge of CMS, DEA, and FDA guidelines.
• Proficient with electronic medical records and pharmacy systems.
• Strong clinical judgement and communication skills.

Responsibilities

• Evaluate orders for appropriateness, dosage, interactions, and route.
• Provide clinical recommendations to physicians and staff.
• Maintain resident profiles and current medications.
• Order and review tests for high-risk medications.
• Collaborate with physicians, nurses, and teams.
• Document all communications and interventions.
• Enforce safe handling and dispensing policies.
• Ensure compliance with state/federal regulations.
• Report medication errors to the Pharmacy Manager.
• Educate clinical staff on safe med handling.
• Assist with other pharmacy tasks as assigned.
