Qualifications

• Bachelor's degree in Pharmacy required; PharmD preferred.
• One year experience in long term care desirable.
• Active pharmacist license in state of practice.
• Knowledge of drug therapy guidelines and regulatory standards.
• Proficiency with electronic medical records and pharmacy systems.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and healthcare staff on therapy changes.
• Maintain up-to-date patient/resident profiles and medication records.
• Ensure laboratory monitoring for high-risk medications is ordered and reviewed.
• Collaborate with physicians, nurses, and other healthcare professionals to support clinical care.
