PharMerica is hiring a PRN Pharmacist in Indianapolis, IN to join a non-retail, closed-door pharmacy serving long-term care and senior living clients.

The role emphasizes clinical decision-making, collaboration with healthcare teams, and maintaining comprehensive patient profiles with current medications and lab results. Qualified candidates will hold a pharmacy degree, be licensed in Indiana, and have a mindset toward ongoing training and growth.

Qualifications

• Bachelor’s Degree in Pharmacy is required; PharmD preferred.
• One year experience in long term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines and regulatory standards.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage and interactions.
• Provide clinical recommendations regarding medication therapy changes and optimizations.
• Maintain patient profiles with medical history, lab results, and medications.
• Ensure monitoring labs for high-risk medications are ordered and reviewed.
• Collaborate with physicians and nurses for clinical and technical support.
• Document communications and interventions in the medical record.
• Enforce policies for safe handling, dispensing and management of medications.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors to Pharmacy Manager.
• Educate clinical staff on safe medication handling procedures.
