You will review orders, advise on therapy changes, and maintain comprehensive resident profiles while working closely with physicians, nurses, and care teams. This role emphasizes clinical oversight, regulatory compliance, and proactive patient care.

Qualifications

• Licensed pharmacist in the state of practice
• 1 year experience in long term care preferred
• Knowledge of medication safety and regulatory standards

Responsibilities

• Evaluate physician orders for drug appropriateness and dosing
• Provide clinical recommendations to physicians and staff
• Maintain patient profiles with medications, labs, and history
• Ensure monitoring tests for high‑risk meds are ordered and reviewed
• Collaborate with healthcare team to support therapy changes
• Document communications and interventions in medical records
