Qualifications

• Doctor of Pharmacy (Pharm.D.) from ACPE-accredited school.
• 1 year hospital pharmacist experience or PGY1 residency completed.
• PGY1/PGY2 residencies and advanced training preferred.

Responsibilities

• Provides expert medication management to optimize patient outcomes.
• Leads therapeutic drug monitoring and pharmacokinetic dosing practices.
• Leads formulary management, stewardship, and clinical/operational optimization.
