Job summary

Tallahassee Memorial Healthcare (TMH) is a major community hospital in Tallahassee, FL, seeking a dedicated Pharmacist to provide safe, effective, and timely pharmacotherapy for hematology/oncology patients. You will collaborate with a multidisciplinary team to optimize patient care and contribute to pharmacotherapy initiatives.

The role requires a Pharm.D., Florida licensure (or eligibility), BLS, and residency credentials; 1 year of experience, with 2–4 years preferred.

Qualifications

Doctor of Pharmacy (Pharm.D.) degree from an ACPE-accredited school of pharmacy.
Post-Graduate Year 1 (PGY1) Pharmacy Residency completion. Post-Graduate Year 2 (PGY2) Pharmacy Residency (in specialization area) completion.
One (1) year of relevant registered pharmacist work experience.
Two (2) to four (4) years of clinical pharmacy experience.
Licensed or eligible for licensure as a pharmacist in Florida; BLS certification; BCOP and/or BCPS credential(s).

Responsibilities

Responsible for providing safe, effective, and prompt medication therapy to hematology/oncology patients.
Demonstrates I CARE values and collaborates with multidisciplinary teams.
Develops and maintains active clinical pharmacy practice and participates in rounds and clinic coverage.
Assesses literature and leads formulary management and medication therapy optimization.
Participates in QA activities and evaluates pharmacist impact on drug therapy measurements.
