Job summary

Tallahassee Memorial HealthCare (TMH) seeks a licensed pharmacist to provide expert medication management to optimize patient outcomes, safety, and cost-efficient drug use within our 772-bed hospital.

Reporting to the Asst Dir Pharmacy Clin Program, you will lead pharmacokinetic dosing, formulary stewardship, and professional development for staff and caregivers in a multidisciplinary setting.

Qualifications

• Doctor of Pharmacy degree from ACPE-accredited pharmacy school.
• One year hospital pharmacist experience or completion of PGY1 residency in lieu of experience.
• Florida licensure or eligibility to practice pharmacy.

Responsibilities

• Provides expert medication management to optimize patient outcomes and ensure safe, cost-efficient drug use.
• Leads therapeutic drug monitoring and pharmacokinetic dosing practices within the care team.
• Leads formulary management, stewardship, and clinical/operational optimization initiatives.
• Leads education and professional development initiatives for patients, caregivers, and healthcare personnel.
• Performs Pharmacist (JC: 021702) duties as required.
