The role requires a PharmD or equivalent, MI license, and hospital infusion experience; ACLS/PALS/BLS certifications may be required. The site operates daytime hours with potential for updated schedules.

Qualifications

• Bachelor's or Doctor of Pharmacy degree from an accredited college.
• 2 years infusion/hospital pharmacy experience preferred.
• ASHP-accredited residency in Pharmacy Practice preferred.
• MI Pharmacist license required within 90 days.
• ACLS/PALS/BLS certifications within 180 days depending on role.

Responsibilities

• Ensure safety and efficacy of medication use and educate patients/family and healthcare teams.
• Interpret and screen medication orders for agent, dose, route, allergies, and interactions; safely dispense.
• Provide pharmacokinetic-based dosage recommendations to optimize outcomes and manage costs.
• Supervise technicians, residents, interns, and support staff within appropriate scope.
• Engage in ongoing professional education and complete required trainings.
• Educate patients and caregivers to optimize outcomes and appropriate use of medications.
• Ensure services comply with state/federal regulations and accreditation requirements.

About Corewell Health Grand Rapids Hospitals Infusion Therapy - Norton Shores

Corewell Health is excited to bring a new infusion therapy location to Norton Shores and the greater Muskegon community. This new 7-chair infusion clinic will help keep care local by providing a convenient location for patients to receive non-oncology infusion therapies closer to home, while expanding Corewell Health's ability to serve the needs of the communities we care for.

As a brand-new location, this is an exciting opportunity to be part of building a clinic from the ground up. Team members will play an important role in establishing the culture, workflows, and patient experience for this new site. The clinic will initially operate Monday through Friday, 8:30 a.m. to 5:00 p.m. As we learn more about patient volume and community needs, there may be opportunities to expand services into evening and weekend hours.

With a limited Corewell Health presence geographically in the greater Muskegon area and no other Corewell Health infusion location in the immediate community, this clinic will provide an important new connection to our health system for many patients. The Norton Shores team will have the unique opportunity to establish that first connection and create a welcoming, high-quality, patient-centered experience from day one.

Scope of Work

• Maintains accountability for ensuring the safety and efficacy of medication use. Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy and participates in process improvement activities and drug use evaluations. Works with multiple medication automation systems.
• Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions. Safely compounds and dispenses medications for administration to patients.
• Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles; provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
• Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel.
• Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities. Completes required continuous training and education, including department and Corewell Health specific requirements on a timely basis.
• Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications.
• Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.

Qualifications

• Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
• 2 years of relevant infusion/hospital pharmacy experience preferred
• American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice Preferred
• LIC-Clinical Pharmacist - STATE_MI State of Michigan 90 Days required
• LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days required
• CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days required Or
• CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days required Or
• CRT-Basic Life Support (BLS) - AHA American Heart Association 180 Days required Or
• CRT-Basic Life Support (BLS) - ARC American Red Cross 180 Days required
• PGY1 Residency or PGY2 Residency in Ambulatory Pharmacy preferred
