Responsibilities

• Reviews and verifies medication orders with patient-specific parameters and clarifies unclear orders with the prescriber.
• Documents and communicates pertinent information to ensure continuity of care.
• Provides drug information to physicians, nurses and other health care professionals.
• Supervises students, technicians, and residents within the pharmacist area of practice.
• Promotes patient safety by submitting medication-related events and near misses.
• Performs daily pharmacy surveillance tool monitoring and other patient care initiatives.
• Works collaboratively with pharmacists and pharmacy staff to maintain a respectful team environment.
