Job summary

NorthBay Health is seeking a Clinical Pharmacist II to provide clinical judgment and pharmaceutical care within the Pharmacy Department. The role includes reviewing orders, counseling staff and patients, and ensuring accurate documentation in the EHR.

The position requires a PharmD or BS in Pharmacy with California licensure and at least one year of hospital-based experience, with potential for shift supervision and special project contributions.

Qualifications

• Pharm D. or Pharm D equivalent or BS in Pharmacy with continuing education.
• California pharmacist licensure required by the state board.
• One year hospital-based experience in unit-dose distribution/IV additives or residency.

Responsibilities

• Review drug orders, patient profiles, and records to evaluate drug therapy appropriateness.
• Select, compound, prepare, and dispense medications per procedures and laws.
• Oversee shift workflow and support technicians and interns.
• Consult with medical staff, nursing, patients on drug therapy and act as drug information resource.
• Maintain accurate patient profiles and perform MAR comparisons.
• Document clinical interventions and formulary compliance in the EHR.
• Identify ADRs and medication errors per policy; initiate adjustments under protocols.
