Job summary

NorthBay Health in Vacaville, CA, is seeking a Clinical Pharmacist II to provide expert pharmacotherapy and patient-facing care within our Pharmacy Department. The pharmacist will review orders, optimize drug therapy, and support a busy hospital workflow, while collaborating with clinicians and nursing staff to ensure safe, effective medication use.

Qualifications

• Doctor of Pharmacy (Pharm D.) or Bachelor of Science in Pharmacy with ongoing continuing education.
• California Pharmacist licensure required.
• One year hospital-based experience in unit-dose distribution and IV additives, or hospital residency.
• Excellent written and verbal communication skills for drug information and therapeutics.

Responsibilities

• Review drug orders, patient profiles and records to evaluate therapy appropriateness.
• Select, prepare and dispense drugs per procedures and regulations.
• Oversee shift workflow and supervise technicians and interns.
• Consult with medical staff, nursing staff, patients and others on drug therapy.
• Maintain accurate patient profiles per procedures.
• Perform MAR comparisons and ensure accuracy in records.
• Document clinical interventions and formulary compliance in EHR.
• Identify and report ADRs and medication errors.
• Under approved protocols, initiate or adjust dosages or labs when needed.
• Assist with Quality Improvement data collection and other assigned duties.
