Qualifications

• PGY-1 Residency OR 1-3 years relevant hospital experience.

Responsibilities

• Initiates and ensures quality of pharmacy and drug distribution services in patient care areas.
• Reviews and verifies medication orders with patient-specific factors and clarifies unclear orders with the prescriber.
• Documents and communicates information with the healthcare team to ensure continuity of care.
• Provides drug information to physicians, nurses and other health care professionals.
• Supervises students, technicians, and residents within the pharmacy.
• Promotes a culture of patient safety by reporting medication-related events and near misses.
• Participates in daily pharmacy surveillance and relevant reports.
• Works collaboratively with pharmacy staff and participates in onboarding and training.
