Duke University Hospital in Durham, North Carolina, seeks a Clinical Pharmacist for PRN coverage. The role provides pharmaceutical care, reviews medication orders, and supports education for staff, patients and students within the Duke Health System.

The position involves rotating shifts and practical oversight of medication processes. Applicants should hold a Doctor of Pharmacy or BS in Pharmacy with appropriate licensure in North Carolina.

Formación

• Ability to communicate clearly in oral and written form.
• Extensive knowledge of drug products, medical therapeutics, pharmacokinetics, and disease states.
• Ability to work with information systems, automated and technical equipment.
• Ability to establish and maintain working relationships with health care teams.

Responsabilidades

• Provide pharmaceutical care services to patients within the Duke Health System including oversight, monitoring, preparation, dispensing and documentation of medication therapy.
• Advise physicians on drug usage and participate in education programs for Pharmacy and Medical Staff.
• Review patient medication orders for safety and efficacy and document interventions.
• Prepare, label and dispense medications in accordance with prescriptions and regulatory requirements.
• Maintain records of dispensing and oversee medication charging systems.
• Educate caregivers, patients and students regarding medications and proper use and monitoring.
