Option Care Health, the nation's largest home infusion provider, is seeking a Clinical Pharmacist in Jonesboro, AR. This entry-level position supports home care patients with safe, effective therapy, education, and coordination with Nursing and other teams to ensure optimal outcomes.

The role emphasizes preparing and dispensing medications, monitoring therapy, and maintaining compliance with state and federal regulations.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: at least one year of experience in sterile IV admixture procedures, knowledge of non-sterile compounding/immunizations/MTM, ASHP residency, APPE, or equivalent experience.

Responsibilities

• Determines suitability of patients for home care.
• Collects and organizes patient information for home care suitability.
• Designs and monitors patient-specific pharmacy care plans for home care patients.
• Provides medication-use education to staff, patients, and caregivers.
• Ensures continuity of pharmaceutical care to and from home and other settings.
• Prepares and dispenses medications following policy and DEA regulations.
• Maintains accurate inventory and adherence to safety and quality assurance.
