Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: sterile IV admixture experience, MTM, residency program, or equivalent.

Responsibilities

• Determines suitability of patients for home care.
• Collects and organizes patient-specific information for home care.
• Designs and monitors pharmacy care plans for home care patients.
• Provides medication-use education to staff, patients and caregivers.
• Prepares and dispenses medications following policy and procedures.
• Ensures compliance with state/federal laws and DEA regulations.
