Job summary

Option Care Health is hiring an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services for home care patients. You will coordinate with Nursing, educate patients and caregivers, and ensure continuity of pharmaceutical care across settings.

This role emphasizes learning in home-care pharmacotherapy, drug information, and adherence to regulatory standards while contributing to a compassionate patient-focused team.

Qualifications

Graduate of an accredited School of Pharmacy.
Current, active Pharmacy license as granted by the State Board of Pharmacy.
One of the following: at least one year of experience in sterile IV admixture, non-sterile compounding, immunizations or MTM, ASHP residency or equivalent experience.

Responsibilities

Assesses home care suitability and gathers patient information.
Designs and monitors patient-specific pharmacy care plans for pharmacotherapy.
Modifies plans based on monitoring data and lab values.
Provides medication-use education to staff, patients and caregivers.
Ensures continuity of pharmaceutical care across settings and proper dispensing.
Maintains equipment, inventories and safety/compliance with regulations.
