Option Care Health in New Orleans, LA is seeking a Clinical Pharmacist for an entry-level role in home care. You will coordinate patient services with the Nursing Department, monitor progress toward goals and educate patients, caregivers, staff and other professionals regarding medication use.

Responsibilities include determining home care suitability, designing care plans, ensuring safe dispensing, and maintaining compliance with state/federal regulations; travel up to 5% may be required.

Qualifikationen

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of: sterile IV admixture experience, non-sterile compounding, immunizations, MTM, residency, APPE, or equivalent.

• Determines suitability of patients for home care and collects necessary information.
• Designs, monitors and evaluates patient-specific pharmacy care plans.
• Provides medication-use education to staff, patients and caregivers.
• Prepares and dispenses medications following policies and regulations.
• Ensures continuity of pharmaceutical care across settings and maintains compliance.
