Job summary

Option Care Health is seeking a Clinical Pharmacist to provide safe, appropriate pharmacy services in a home care setting. This entry-level role collaborates with Nursing and other professionals to educate patients and caregivers and to ensure optimal pharmacotherapy outcomes.

Responsibilities include designing care plans, monitoring progress, dispensing medications, and adhering to regulatory standards while maintaining patient confidentiality. Travel up to 5% may be required.

Qualifications

• Must be a graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• Experience in sterile IV admixture procedures or knowledge of non-sterile compounding, immunizations and MTM.
• Completion of an ASHP‑accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience preferred.
• Willing to travel up to 5% of the time for business purposes within state.

Responsibilities

• Assess patient suitability for home care and collect relevant information.
• Design, monitor and evaluate patient-specific pharmacy care plans for home care.
• Provide medication-use education to staff, patients and caregivers.
• Ensure continuity of pharmaceutical care between home care and other settings.
• Prepare and dispense medications following policies and procedures and comply with DEA regulations.
• Maintain confidentiality and comply with safety and quality standards.
