Option Care Health, Inc. is seeking a Clinical Pharmacist to provide safe, appropriate pharmacy services in a home-care setting. The role is entry-level within clinical pharmacy, coordinating with nursing staff and educating patients, caregivers and professionals on medication use.

The position emphasizes patient-centered care, adherence to USP standards, DEA regulations, and maintaining perpetual inventory control while ensuring quality and safety across all processes.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• Experience or training in sterile IV admixture or MTM is preferred.

Responsibilities

• Determines suitability of patients for home care and collects patient information.
• Designs and monitors patient-specific pharmacy care plans for home care.
• Educates patients, caregivers and staff on medication use and safety.
• Ensures continuity of pharmaceutical care across home and other settings.
• Maintains inventory, dispensing accuracy and regulatory compliance.
• Responds to drug information requests and participates in CEU opportunities.
