Job summary

Option Care Health, a leading home infusion provider, seeks a Clinical Pharmacist to deliver safe, evidence-based pharmacy services in the home care setting.

This entry-level clinical pharmacist role coordinates with Nursing, educates patients and caregivers, and ensures high-quality pharmaceutical care in accordance with regulatory standards. Strong foundation in pharmacotherapy and patient communication is essential.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• Experience providing customer service to internal and external customers.
• Willingness to travel up to 5% of the time for business purposes (within state).
• Preferred qualifications include completion of ASHP residency or APPE and experience in home care.

Responsibilities

• Determines the suitability of individual patients for home care.
• Collects and organizes patient information needed for home care suitability.
• Assesses patients for home care per Option Care policies.
• Designs and monitors patient-specific pharmacotherapy care plans for home care patients.
• Provides medication-use education to patients, caregivers, and staff.
• Ensures continuity of pharmaceutical care across home and other care settings.
• Prepares and dispenses medications following policy and safety standards.
