The position requires a pharmacy degree, state licensure or reciprocity eligibility, and relevant certifications. Weekend rotation and varied shifts are part of the schedule in a high-acuity environment.

Qualifications

• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school or an FPGEC certificate.
• Licensed in the state(s) of practice or eligible for reciprocity.
• Immunization and non-vaccine injectable certification required based on area of practice.
• Credentialing and privileging required based on area of practice.
• No experience required.

Responsibilities

• Oversees pharmaceutical care and designs medication therapy regimens using biomedical literature.
• Provides counseling to patients and caregivers on medication therapy, adverse effects and usage.
• Interprets and reviews medication orders and prescriptions accurately.
• Participates in management of medical emergencies and formulary adherence.
